Forms and Requirements


Students age 16 and above may participate in ETC programs, through referral by the high school outreach counselor or other service agency. (Note: The student/parent agreement form is required of all high school students or students under age 18.)

Medical insurance is required for all students entering an ETC program. Students without medical insurance may elect to purchase student accident insurance through ETC. Refer to the individual program page for specific costs.

In addition, the following forms and clearances must be submitted to ETC Student Services prior to meeting with an ETC counselor to register for the program.
